Overview

Chilton Medical Center, a member of Atlantic Health System,is seeking a Full-timeNursing Coordinatorfor O/P Oncology/Breast cancer navigation srvc.working primarily 8 :00 AM - 5:00 PM.

Responsibilities

Essential Job Functions:

 

Primary focus will be on breast cancer research & patient navigation:

 

  • Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served.
  • Candidate must have excellent communication and computer skills.
  • Coordinates Patient participation on clinical trials.
  • Ascertains with physician that potential protocol patient has met the eligibility and pretreatment requirements of the protocol, and that informed consent has been obtained prior to registration.
  • Monitors treatment calculations, dosage modifications and patient toxicities on a regular basis.
  • Facilitates scheduling of patient visits specific to protocol; communicates and collaborates with all departments involved.
  • Obtains pathology or other lab materials and completes appropriate forms when needed and data from other outside sources.
  • Establishes processes and procedures to effectively identify and manage cancer patients across the entire continuum of care (from diagnosis through treatment and survivorship)
  • assists with the development, implementation and adherence of clinical pathways coordinates patient care with all healthcare providers,
  • contributes to the development, implementation, evaluation and revision of policies and procedures related to cancer care
  • provides patient-specific educational information and resources to cancer patients in order to assist patients and families with coping and understanding of their diagnosis and treatment
  • Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers

Qualifications

Education:

  • BSN Preferred

 

Required License:

  • Valid New Jersey Nursing License Required

 

Certifications:

  • OCN Required
  • CCRC Preferred

 

Related Experience:

  • 3 + years of Oncology experience as a Registered Nurse in a Hospital/physician practice setting required.
