    Roselle Park Medical Associates, part of Atlantic Medical Group is currently seeking an experienced Nurse Practitioner.

    Responsibilities

    Essential Functions:

    • The Nurse Practitioner will perform comprehensive assessments for all patients, order medications, treatments, monitoring for efficacy and for possible side effects, order lab and other diagnostic tests, including follow up for results, communicate with the collaborating physicians on a regular basis per collaborative practice agreement, travel to nursing homes/ assisted living facilities/ homes to deliver care to the practice patients, consult with patients and families regarding goals of care; follow patients through various transitions of care, to and from hospitals, facilities, and home, coordinating with in-hospital staff, facility staff, and home care agencies to ensure continuity of care. In addition the NP will coordinate chronic care programs for the practice, educate patients and families, specifically on disease process and medications, collaborate with the interdisciplinary team, provide comprehensive documentation in electronic medical record/ and /or paper medical records, assist patients and families with long term planning and resources for in home care, answer telephone calls from office, pharmacies, caregivers, patients and families, and facility staff, and manage acute and chronic illnesses.

    Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.

    Qualifications

    Education:

    • BSN, Master's Degree required.

    Qualifications/Requirements:

    • Valid NJ RN License, BLS, ACLS, and minimum three (3) years experience in Acute Care, Palliative Care/IP Hospice, Nursing Home, Geriatric, or Wound Care required
    • Excellent communication and computer skill essential
