  • Overview

    Atlantic Medical Group, member of Atlantic Health System, is seeking a full-time Social Worker for Palliative Grant.


    The MSW will serve as a member of a multidisciplinary palliative care team with liaison responsibilities with hospital and staff members of community organizations and other health care organizations.


    Essential Functions:

    • The Social Worker will be responsible for independently conducting psycho/social assessment/evaluations, participating in professional team review of patient care, providing individual, family and group counseling, identifying community resources and assist patients and their families in connecting to these resources, facilitating family meetings, working with anticipatory grief and difficult family dynamics, and understanding placement options and appropriate social, financial and legal services.



    • Master's in Social Work from an accredited school of Social Work.


    • LSW or LCSW required
    • Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) preferred
