Overview

Atlantic Visiting Nurse has an exciting opportunity for a Clinical Coordinator in Hospice.  This is a dual role consisting of providing education and mentoring of hospice staff in classroom and field settings as well as visiting hospice patients in their place of residence.

 

Atlantic Visiting Nurse, part of Atlantic Health System, provides high quality care to patients and families in the comfort of their own homes, facilities, and hospitals.  We are certified by The Joint Commission and available to residents of Morris, Union, Essex, and Somerset counties New Jersey.

Responsibilities

Essential Job Functions: 

  • Providing education to hospice nurses and other members of the hospice team in a classroom and field setting while maintaining skills by visiting patients in their place of residence 2 to 3 days a week
  • Maintain current knowledge of clinical issues and regulatory and reimbursement standards

Qualifications

Education: 

  • Graduate of an accredited Nursing Program required
  • BSN Preferred

 

License: 

  • Valid New Jersey Nursing License required
  • CHPN preferred

 

Qualifications/Requirements: 

  • A thorough knowledge of the Medicare Hospice benefit including admission criteria, philosophy and services
  • Experience in educating, mentoring, and precepting hospice staff to assist them in being successful and knowledgeable in their roles
  • Expert pain and symptom management skills
  • Knowledgeable in the signs and symptoms associated with end of life
  • Excellent verbal and written communication skills

 

Relevant Experience: 

  • 3 to 5 years of experience as registered nurse in Hospice, Palliative Care, or related field
  • 2 to 3 years of experience in education


Hours: 

  • Fulltime: 8am-4pm (Monday through Friday)
