The Application Systems Analyst II – Healthy Planet brings value to the Population Health Team by driving member usage and engagement with Quality Reporting and Healthy Planet tools across all AHS organizations. This position is responsible for working directly with department leaders, Physicians, Nurses and clinic office staff to assist with effective usage, engagement with, and design of Healthy Planet and Quality Reporting tools and programs.


  • Manage and drive engagement strategies to increase member usage of Healthy Planet tools
  • Manage and increase member involvement in Quality Reporting tools for Promoting Interoperability (MU, MIPS and UDS)
  • Manage and communicate with AHS members, customers, and partners regarding Population Health and Quality Reporting program activities in a clear and customer-centric manner
  • Provide one-on-one support for AHS members and customers requiring assistance in Population Health related areas
  • Engage Account Managers and MCT analysts in member outreach calls and communications as they relate to Population Health
  • Manage, create, and continuously update program communication documents for both internal and external customers
  • Facilitate member engagement forums and workgroups • Provide insight, improvement ideas, and critical thinking to Population Health and Analytics team
  • Research, identify, and implement best practices and improvement ideas in support of Population Health programs and goals
  • Perform outreach and build relationships with AHS members and customers in support of the Population Health deliverables
  • Act as a point of communication for questions and inquiries about the program, internally and externally
  • Actively seek opportunities for the program to bring increased value to AHS members, patients, partners, and staff
  • Cultivate, build, and maintain relationships with internal and external program stakeholders


Bachelor's degree in Healthcare Administration, Public Health, Mathematics, Healthcare Informatics or Computer Science or equivalent experience

  • Minimum of three (3) years of experience in similar or relevant role
  • Four (4) year degree in healthcare administration, business administration, public administration, program or project management, IT, health informatics, or a related field or other professional experience that meets the technical requirements of the position
  • Experience facilitating and coordinating meetings with various diverse functional groups and stakeholders in person and online
  • Experience defining appropriate metrics necessary to determine if a change is an improvement and using data analytics to measure progress and project success
  • Experience with manipulating large worksheet files.
  • Experience using Epic electronic health record platform as an end-user, trainer, or change control agent in Ambulatory setting is a plus.
  • Familiarity with improvement and evaluation methodologies and tools such as the Model for Improvement and/or Lean
  • Familiarity with ACO, as well as ability to communicate with ACO and Integrated Care/Care Coordination leadership
  • Technical aptitude to fully understand and obtain the knowledge required in order to effectively collaborate with AHS’s members and customers
  • Demonstrated experience with program or project management or equivalent experience
  • Proven success in internal and external relationship management with all levels of expertise from administrative to executive
  • Technical aptitude to fully understand and obtain the knowledge required in order to effectively collaborate with members
  • Strong training and/or presentation skills • Excellent interpersonal, intercultural, and customer service skills
