Overview

Atlantic Health System is currently seeking a per diem Social Worker for Continuum Care Department at Morristown Medical Center.

 

Hours are 8am-4pm, must be available for 3 shifts per week, shifts are not guaranteed, must provide avaliability 3 months in advance. 

 

  • 2 -3 shifts/per week required; shifts not guaranteed.
  • Weekend "only" requirement:  Must be available to work 2-3 w/e shifts per/month.

Responsibilities

Essential Functions/Responsibilities of Position:

  • Responsibilities will include discharge planning for post-acute needs, insurance authorizations for sub-acute rehab, maternal child services and LTAC facilities, conducting psychosocial assessments, family meetings, and acting as an integral part of reducing length of stay team.

Qualifications

Minimum Education Required:

  • Master's in Social Work from an accredited school of social work

Minimum Experience Required:

  • NJ Social Work License LSW or LCSW required.
  • Previous hospital social work ecperience preferred.
  • Familiarity with Epic preferred.
