Social Worker, Care Coordination

Atlantic Health System, is seeking a Full-time Social Worker in the Care Coordination Department to work 8A-4P and weekends as needed.





The MSW Social Worker works in collaboration with a multidisciplinary team of clinicians as part of the Care Coordination program to support high quality care for patients and families in alignment with Atlantic Health PRIDE values.

50% - Patient assessment, identification of need, and making referrals
30% - Documentation
10% - Meetings, program development
10%- Collaboration with inside/outside agencies


4. Job Functions:

Job Function 01: Referrals and Service Identification
Responds to all referrals in accordance with Care Coordination time frames.
Participates with patients in the ED/OBS, IP and ambulatory settings to introduce Care Coordination Services, complete Social Determinants of Health assessment and CHW assessment, identifies and prioritizes barriers to care in conjunction with an LSW/LCSW and works with physicians, clinicians and patient/family to set achievable health care goals.
Maintains and utilizes a current knowledge base of community agencies and facilities.
Refers patients/families to appropriate level of care (LSW, LCSW, Nurse Care Coordination, Physician) to assist in coordinating services.
Provides appropriate follow-up with patient/family on their efforts to follow through with referrals.
Assists patients with transportation services, applications, locating and referring to local community resources and facilitates referrals to the Care Coordination team.
Meets patients in the ED/OBS, IP, home setting and PCP office to assist in achieving health care goals as determined by clinical staff.

Job Function 02: Collaboration

Ongoing collaboration and communication with Care Coordination team members/interdisciplinary team to update and reassess changing psychosocial needs, medical needs, and plan of care.

Proactively communicates with the Care Coordination Clinical Leadership team regarding situations that interfere with the implementation of the patient s care plan.

Act as liaison and patient advocate to community agencies, health care delivery system services and third party payers, when indicated.


Job Function 03: Professionalism and Departmental Development

Utilizes constructive insights, feedback & criticism for improved performance, professional awareness and development.

Regularly attends and actively participates, in an appropriate manner, assigned intra-disciplinary and interdisciplinary forums, administrative meetings, staff meetings, supervisory sessions, and in-service training.

Annually participates in a minimum of 3 educational programs on topics relevant to practice area.

Job Function 04: Documentation

Responsible for timely documentation as directed by Social Work Manager.



  • Master of Social Work or related field is required.
    6. Required Licenses, Certifications and/or Registrations
    Licenses MSW Preferred

    7. Other Desired Skills, Abilities, and Knowledge (Maximum of 10 bullet points)
    Ability to function within an interdisciplinary team
    • Ability to identify patient need through excellent assessment skills
    • Ability collaborate with community agencies
    • Ability to act as advocate for patients


    Health Coaching related courses or certification a plus.

    Experience and knowledge of local and county resources.


  • Current NJ LSW required.

Relevant Experience:

  • Experience 3-5 years in health related field, hospital experience preferred
    Strong knowledge of Community Resources
    Strong interpersonal, priority setting, problem solving, attention to detail and organizational/time management skills.
    Ability to navigate EHR and data bases with demonstrated ability to electronic documentation.
    Clear and accurate verbal, telephonic, and written communication skills.
