Overlook Medical Center is currently seeking a clinical pharmacist within the Pharmacy Department.
This position will be a full-time inpatient transitions of care (TOC)/internal medicine (IM) clinical pharmacist for Overlook Medical Center who will collaborate with and serve as a resource for nurses, physicians, pharmacists, and medical residents. The TOC/IM clinical pharmacist will participate in daily multidisciplinary rounds on a dedicated unit, assist in discharge medication reconciliation, and provide patient education. In addition, the TOC/IM clinical pharmacist will work with a team of TOC/IM clinical pharmacists to support providers and case management throughout the hospital with patient access to medications (including participation in enrolling eligible patients in grants). The clinical pharmacist will participate in guideline and policy development and additional system initiatives. The clinical pharmacist and will also be responsible for providing education for pharmacy, medical providers, and nursing staff, and will serve as the pharmacy representative for relevant committees.
