Overlook Medical Center is currently seeking a clinical pharmacist within the Pharmacy Department.

This position will be a full-time inpatient transitions of care (TOC)/internal medicine (IM) clinical pharmacist for Overlook Medical Center who will collaborate with and serve as a resource for nurses, physicians, pharmacists, and medical residents.  The TOC/IM clinical pharmacist will participate in daily multidisciplinary rounds on a dedicated unit, assist in discharge medication reconciliation, and provide patient education. In addition, the TOC/IM clinical pharmacist will work with a team of TOC/IM clinical pharmacists to support providers and case management throughout the hospital with patient access to medications (including participation in enrolling eligible patients in grants). The clinical pharmacist will participate in guideline and policy development and additional system initiatives. The clinical pharmacist and will also be responsible for providing education for pharmacy, medical providers, and nursing staff, and will serve as the pharmacy representative for relevant committees. 

Essential Functions:

• Function as a unit-based clinical pharmacist as a member of the interprofessional team who proactively contributes to the decision-making process and the development of patient-centered care plans
• Deliver satisfactory and valued services and finding mechanisms to reduce medication costs
• Provide responses to drug information questions
• Counsel patients on new medications and provide discharge counseling
• Provide transitions of care services for patients throughout the hospital
• Contribute to system-wide TOC/IM-focused clinical initiatives including guidelines, policies, and formulary management
• Advise clinicians on drug selection based on current clinical guidelines to optimize drug therapy  
• Identify, report, and analyze medication errors and adverse drug reactions
• Precept pharmacy practice residents and pharmacy students
• Participate in quality, safety and other project work related to TOC/IM
• Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with publication and/or presentation at local and/or national meetings
• Assists in the development of policy and procedures
• Participate in multi-disciplinary committees including Pharmacy & Therapeutics, Medication Management, and specific committees to support the appropriate use of medication therapy

Requirements/Education/Licensure:

• Current NJ Board of Pharmacy License
• Doctor of Pharmacy (PharmD)
• PGY-1 pharmacy practice residency

Additional Qualifications / Requirements

• Board Certified Pharmacotherapy Specialist (BCPS) preferred
• PGY-2 Pharmacotherapy or Internal Medicine residency preferred
