Overview

Morristown Medical Center, a member of the prestigious Atlantic Health System is currently seeking a PD Case Manager in the Case Mgmt. - State Dept.

Responsibilities

We provide comprehensive case management including assistance with medical systems, community/State/Federal resources, advocacy with school issues and other entities that impact on children with special health care needs.


We provide case management for children from birth up to their 22nd birthday, with a special health need.

 

Qualifications

Licensure: Must have a valid New Jersey nursing license (RN) OR valid New Jersey certification/license(CSW/LSW/LCSW)


Education: Must have a minimum of a Bachelor or Master degree in nursing (BSN/MSN) OR a Bachelor or Master degree in social work (BSW/MSW).

 

A successful candidate must have good assessment and problem solving skills, knowledge of community resources and good communication skills by phone.

 

Experience: Must have a minimum of one year of experience in pediatric case management for “Children and Youth with Special Health Care Needs”.
