Atlantic Health System is seeking a full- time Social Worker for Case Management Department  to work 8 hour shifts between the hours of 8a-6p and 1 Sat. per month as needed.


Atlantic Health System is seeking a part time Social Worker at Overlook Medical Center.


  • Social Worker provides psychosocial assessments, counseling and facilitation of discharge plan in all areas within the hospital. Responsibilities include:
  • Conducting psychosocial assessments
  • Discharge planning for post acute needs
  • Obtaining insurance authorizations
  • Making referrals to appropriate community resources
  • Active participation in family and team meetings
  • Providing supportive counseling
  • Crisis intervention
  • Assessing and making referrals for abuse and neglect (adult and pediatric), providing support and counseling at the end of life




Minimum Education:

  • MSW required


  • Current LSW or LCSW license in the State of New Jersey

Relevant Experience:

  • 1 year previous hospital SW experience and/or acute care experience preferred (knowledge of systems - financial, insurance, advocacy, legal, knowledge of community resources)
  • Familiarity with Epic preferred
