Overview

Atlantic Behavioral Health's Valor Health Connection Program - This is a grant funded position to ensure access to integrated healthcare for Veterans, active military and first responders. The Project Coordinator will provide leadership to help implement this new program and supervise a team of behavioral health professionals to ensure the program delivers exemplary service to Veterans, active military and first responders and their families as they seek to navigate the healthcare system.

Responsibilities

Essential Responsibilities:

  • Implement program specific policies and procedures and ensure AHS policies and procedures are followed.
  • Provide leadership to a comprehensive multidisciplinary team.
  • Responsible for the day-to-day operation of the program.
  • Responsible for grant management components such as data collection and reporting requirements.
  • Establish strong relationships within the VA Systems and community stakeholders to coordinate and ensure a comprehensive approach to accessing mental health and medical care.
  • Coordinate outreach to Veterans, active military and first responders to ensure they are aware of the services available and assess community gaps and service needs.
  • Responsible for providing leadership to case managers to assess social determinants of health needs and navigate the VA medical system, benefits, insurance, Medicare, Medicaid and other community aid requirements.
  • Responsible for overseeing participant assistance funds to provide emergency assistance and wrap around funds to participants.
  • All other duties as assigned.

Qualifications

Requirements:

  • Must possess a master's degree in a social service-related field
  • Licensure preferred (LPC, LCSW, LAC, LSW)
  • Coursework in addictions and/or a dual licensure in mental health and addiction licensure (LPC/LCADC, LCSW/LCADC) preferred
  • Three years minimum experience in a related field
  • Veteran Status and/or experience working with Veterans, active military and first responders preferred.
  • Must have knowledge of the VA system
