Overview

Atlantic Health System is seeking a Full-time Mental Health Assistant in the Out-patient Mental Health Unit at Morristown Medical Center to work 8:30am - 4:30pm.

Responsibilities

Essential Functions/Responsibilities:

  • Help identify and make bedside rounds to inpatient Veterans.
  • Meet with Veterans identified and referred from outpatient departments.
  • Coordinate care with heath-care teams to assist in linkage to care in the community.
  • Act as patient advocate to assist Veterans with overcoming barriers.
  • Provide educational information and resources to patients and families.
  • Coordinate outpatient services.
  • Ensure linkages.
  • Develop relationships with system partners to increase awareness of behavioral health.
  • Other duties as assigned.

Qualifications

Minimum Education:

  • Bachelors Degree in Psychology, Social Work or other related fields w/ relevant experience
  • Master preferred.

Minimum Experience:

  • 2+years case management in behavioral health experience
  • Experience of working with and understanding of community resources for Veteran's
  • Experience working on an inpatient medical unit a plus
