Atlantic Health System is seeking a Per Diem pharmacist in our Retail Pharmacy at Newton Medical Center.
Working knowledge, is responsible to ensure the workflow of the retail pharmacy is maintained in an efficient manner. Ensure presciptions are filled in a timely manner, including the accurate maintenence of patient profiles. Ensures that all prescriptions are adjudicated and rejections are acted upon. Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant labs, allergies, height, weight and BSA, pregnancy and lactation status. Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency. Makes recommendations for alternate treatments when necessary. Dispenses medications, prescriptions, miscellaneous items accurately and promptly. Processes requests via phone, window, fax and computer in a professional and timely manner. Obtains medications when needed to provide optimal patient care using economical or alternative options. Legibly signs all paperwork and/or labeles associated with these processes.
