Atlantic Health System is seeking a per-diem Social Worker for the Case Management Department at Newton Medical Center to work 8AM - 4PM, including 1 weekend shift a month.

Entry level offer starts at $40.11 / hour and increase based upon experience, credentials, education, and roles and responsibilities

Essential Functions/Responsibilities:

• Discharge planning from an acute care setting using a multidisciplinary approach
• Supportive therapeutic counseling 

Minimum Education:

• Bachelor's degree in social work
• MSW

Minimum Experience:

• 1-2 years medical acute care discharge planning experience preferred

License/Certification:

• Licensed (either LSW or LCSW)
